As part of our referral process, you’ll receive exam notes promptly after your patient’s consultation, medical eye exam, or surgery with us. We’ll also provide all necessary billing information for co-managed patients, so the process remains seamless for you and your team.

Referral submission with our online portal:

To submit a referral via our secure electronic portal, please click on the link below and follow the instructions for submission. A representative will process your referring provider information and contact your office with any questions.

If you prefer to fax your referral request, please send the following information to 360-822-3237:

  • Patient demographics (name, date of birth, insurance information, mailing address, phone number)
  • Reason for referral
  • Patient's medical records, medications, and allergies
